When removing a pallet from the fourth level of a rack (approx. 7.7 m high), an employee failed to raise the forks to the correct height. Instead of sliding the forks evenly under the pallet, they were inserted too low and pushed the pallet backwards into the rack. As a result, the pallet tilted and began to fall down on its shorter side, partially leaning against other goods. Instead of stopping work and reporting the problem, the employee tried to correct the pallet himself. To do this, he used a smaller wooden pallet and lifted the tilted pallet from below with the forks. During this maneuver, the forks were raised too high and hit a shelf beam.
