Incident description

While storing goods, an employee was standing on an unstable ladder, which was caused by the lack of a non-slip foot on one of the side rails, making it shorter than the others. In a hasty attempt to climb down from the wobbly structure, he placed his entire weight on his foot, which was incorrectly positioned on the ground.

Incident effects

  1. Sprained ankle.
  2. Employee absence.
  3. Potential risk to employee safety: similar incidents in the future could lead to serious injuries due to falls from height when using ladders.

What can you do to avoid an accident?

  1. Each time before using a ladder or platform, check their technical condition for damage and defects, in particular: non-slip feet, steps, and folding safety devices.
  2. Report any non-compliance related to the technical condition of ladders and platforms to your supervisor immediately.
