Incident description

On the night of November 20-21, 2025, at 1:57 a.m., the operator ran over the warehouse worker's right foot with the left wheel of the forklift.

At the time of the incident:
The passageway was blocked, visibility was impaired. In such conditions, both the warehouse worker and the operator failed to exercise increased caution. In addition, the operator was operating a forklift truck for which he did not have the required UDT (Technical Inspection Authority) license, and access to this forklift truck was provided to him by another operator.

Incident effects

  1. Injury to the right foot, requiring immobilization of the limb.
  2. Employee excluded from work for the duration of treatment.
  3. Failure to comply with procedures regarding forklift movement, accessibility of passageways, and operator qualification rules.
  4. Danger to other employees in the area with limited visibility.

What can you do to avoid an incident?

  1. Do not enter the forklift's working area if the space is not safe. Make sure you have eye contact with the operator and that they can see you before entering the driving path. Maintain a principle of limited trust—because the forklift may change its path or stop with a delay.
  2. Do not share your license with another person and do not operate a forklift truck for which you do not have a license.
  3. In conditions of poor visibility, use the horn and slow down.
