WHO CAN OPERATE THE TRUCK:
To operate the forklift, an employee must:
- be at least 18 years of age,
- have a valid medical examination (with the note "for operating forklifts"),
- have UDT certification or other relevant training,
- receive a personal internal permit from the employer.
SCOPE OF APPLICATION:
The procedure applies to all company employees and operators from external companies and employment agencies who operate forklifts on the premises.
REQUIREMENTS FOR FORKLIFT OPERATORS
1. Lifting trucks (push trucks, tractor trucks – without a lifting mechanism):
- Medical examinations: certificate for operating forklifts, working at heights up to 3 m.
- Licenses: category B driver's license or certificate of completion of training appropriate for the type of forklift.

Operation of lift trucks, excluding specialized ones:
- Medical examination: certificate for operating trucks, working at heights up to 3 m.
- Qualifications: UDT certificate for lift trucks (former categories II WJO, III WJO – e.g., front, side, guided).

Forklift operation, including specialized forklifts:
- Medical examinations: certification for operating forklifts, working at heights above 3 m.
- Qualifications: UDT certificate for lift trucks, including specialized ones (former category I WJO).

EMPLOYEES OF EMPLOYMENT AGENCIES:
- Must be employed as forklift operators.
- They may only operate lifting and forklift trucks, not specialized ones.
- The agency must provide qualifications and a medical certificate.
- The rest of the procedure is the same as for other employees.
STAGES OF OBTAINING A PERMIT:
1. Application in JIRA:
Please provide:
- first name, last name, company,
- scans of qualifications (UDT, training, driver's license),
- information on whether the course was internal.
2. Verification of documents by health and safety service employees:
- The employee provides the original documents.
- If there are no tests, the health and safety service employee issues a referral to a doctor.
3. Theoretical health and safety training:
- Conducted by an occupational health and safety service employee, it is completed with a test (minimum 12/15 correct answers).
4. On-the-job training (practical):
- Conducted by a supervisor or experienced operator in the presence of the Shift Coordinator.
- Duration: 8 hours.
- Initially, the speed of the forklift is limited to 4 km/h, then 6 km/h.
- After completing the practical training, there is a practical exam.
5. Issuing a personal permit:
- After positive verification, the health and safety officer issues an internal permit.
- Permit colors:
- green – lifting trucks,
- blue – lifting platforms,
- red – specialized.

6. Additional rules:
- Failure to comply with the instructions will result in a mark (hole) on the signal.
